Definition
The TBI-QOL Depression Scale is Patient Reported Outcome assessment of depression. It is made up of items from the PROMIS and Neuro-QOL measurement systems. It can be administered via Computer Adaptive Testing (CAT) or via short forms. With CAT, the participant is administered items one at a time and, with each item, the computer prorates the score estimating the examinees level of depression. Generally, reliable scores can be obtained with as few as 4-5 items. As an alternative to CAT, short forms are also available.

Depression (also known as Major Depressive Disorder) is a common but serious mental health condition that profoundly affects a person’s feelings, thoughts, and actions. It’s more than just feeling “sad” or “blue” for a day or two.
What it Is
At its core, depression is characterized by a persistent, deep feeling of sadness and/or a significant loss of interest or pleasure in almost all daily activities (like hobbies, socializing, or work).
How it Affects a Person
For a person to be experiencing depression, these feelings must last for a minimum of two weeks and significantly interfere with their daily life. Other common signs and symptoms often experienced include:
- Changes in energy and sleep: Feeling extremely tired (fatigue) or having trouble sleeping (insomnia) or sleeping too much (hypersomnia).
- Changes in appetite: Significant weight loss or gain.
- Difficulty concentrating: Trouble making decisions, focusing, or remembering things.
- Negative self-talk: Feeling worthless or overly guilty.
- Physical changes: Feeling restless or slowed down to an extent noticeable by others.
- Thoughts of death or suicide.

Details
The TBI-QOL Depression CAT yields a standard score on a t-metric (mean of 50 and standard error of 10). It is a continuous variable.
Though the TBI-QOL measure was developed and tested with individuals with TBI, the scores have been equated with those of the PROMIS Depression bank. This will allow direct comparison of TBI-QOL Depression scores with PROMIS depression scores. This will allow the TBIMS depression scores to be comparable to other studies that include PROMIS Depression.
QOL questions should not be administered to a significant other, or any other proxy. If the individual is unable to provide data, use code “10. Not Applicable: No data from person with TBI.”
If administering the Computer Adaptive version, preface questions with: “This next section will be entered directly into the computer and cannot be changed. Please think about your responses carefully before answering.”
If the TBI-QOL measures should need to be re-administered for any reason, the NDSC will use the last version submitted.
If clarification is needed repeat the word/question and answer based on what the word/question means to them.
